The Virtual Care Decision Framework has been developed to provide structure and guidance to health professionals and system leaders across disciplines and the continuum of health care as they plan, develop and implement sustainable and scalable systems and processes that fully integrate virtual modalities into care delivery. It incorporates five elements of virtual care.

This toolkit has been updated to support healthcare providers in the delivery of stroke care across the continuum via virtual modalities. Virtual Cardiovascular Prevention and Rehabilitation Implementation Toolkit +

This toolkit was developed in collaboration with the Canadian Association of Cardiovascular Prevention and Rehabilitation (CACPR). It aims to support the delivery of cardiovascular rehabilitation and secondary prevention via virtual modalities by providing guidance and practical tips as health professionals integrate teleconferencing, videoconferencing, secure messaging, or audio digital tools into their practice.
